Q: Why has our name changed to HopeHealth?
A: Providing hope is what we do. For over 30 years, Hospice & Palliative Care of Cape Cod has been providing hope to thousands of people experiencing the impact of serious illness or loss. Now – we ARE Hope.
As HopeHealth, we can help more people gain access to our care and services – earlier and easier. This name change reflects our geographic expansion off Cape Cod, our affiliation with Alzheimer’s Services, and the addition of our new HouseCalls program. All of these changes benefit the communities we serve. They also help to address unmet community needs and ensure the long-term viability of our service to Cape Cod and the surrounding area.
Q. How does the HopeHealth name embody our mission?
A: Our mission is to improve quality of life for everyone in our community who is coping with serious illness. That’s why we created our “Open Door Philosophy”. This philosophy has made our expert hospice care available to every eligible patient -- regardless of the complexity of their care needs, the costs of their care, or their ability to pay. As a result, we have been able to serve many more patients and families. This philosophy of openness and advocacy is integral to our new HopeHealth identity and now spans across all of our emerging service lines.
The commitment to our mission is steadfast, and more relevant today than at any time in our past. It drives the decisions we have made and our strategy moving forward.
Q: Why have we added new services beyond hospice? What do we mean by unmet community needs?
A: People are living longer. Often, they are living for many years with multiple chronic conditions. In order for them to maintain their optimal health and independence, they need sophisticated, coordinated care available where they live. This is particularly true on Cape Cod and the surrounding communities we serve. Cape Cod has one of the oldest populations in the country with more than 10,000 people suffering from dementia or Alzheimer’s disease and over 5,000 homebound residents. In Bristol and Plymouth counties an additional 10,000 are estimated to be home-bound.
We remain committed to serving everyone who is eligible for hospice services and we have extended this commitment to serving those in need of other specialized care.
