HopeHealth Palliative Care provides relief from the symptoms and stress of a serious illness, wherever you need care.
Our team of medical professionals can collaborate with your existing doctors and other healthcare providers to ensure your care is well coordinated.
We can control your pain and other symptoms. And we can help you and your family clarify the goals of your care and convey your priorities.
We provide care in a variety of settings:
Hospital and Outpatient Care
If you or a loved one is hospitalized at Rhode Island Hospital, Miriam Hospital, South County Hospital, Newport Hospital, Sturdy Memorial Hospital, or receiving care at the Lifespan Cancer Centers, we can work with your medical team to provide palliative care. Patients receiving palliative care in an inpatient setting at these partnering hospitals or receiving care at Lifespan Cancer Centers may be eligible to have services coordinated by our care transitions team. Home-Based Care - Advanced Illness Care
What is Advanced Illness Care?
HopeHealth Advanced Illness Care provides palliative care to patients in the later stages of illness. Our program prioritizes quality of life by anticipating, preventing, and treating the symptoms and stress associated with serious illness. Care can be provided alongside curative treatments and patients do not have to meet criteria for skilled home care.
Services include:
- Expert symptom management to reduce physical and emotional distress.
- Skilled communication, especially when difficult decisions need to be made by care teams, patients or families.
- Patient and caregiver education on what to expect as an illness progresses, and how to handle situations at home.
- Goals-of-care conversations to ensure that treatments match the changing needs and priorities of the patient and family over the course of a disease.
- Collaboration with any other services that a patient may have in the home, such as skilled home care and private duty services.
- Social workers and nurses are also available
Who is eligible for Advanced Illness Care?
This program offers support to patients who are facing advanced stages of illness, where their health status and functioning are declining. It aims to provide care and comfort during this challenging time. Eligibility for the program is determined based on compassionate criteria derived from CAIC (Consensus Standards for Community-Based Palliative Care).
Who offers the care?
Our dedicated team of nurse practitioners works closely with your primary healthcare provider to provide compassionate care. We offer consultative services, offering valuable recommendations to support your well-being. It's important to note that in this program, our HopeHealth clinicians do not prescribe medications, ensuring a focused approach to your care.

Hospital, clinic, home: Understanding the settings of palliative care
Long-Term Care Facilities
If you are receiving care in a nursing home or skilled rehabilitation facility, we can work alongside your healthcare team to provide palliative care in that setting.
Pediatric Care (PediPal)
PediPal is our palliative care program for children and families in Massachusetts. Services include:
- Pain and symptom management
- Assessment and case management
- Spiritual care, social services and counseling for the child and other family members, especially siblings
- Complementary therapies, such as music therapy, massage therapy and engagement with a child-life specialist
- Volunteer support, respite care and grief support services for family members
This program is funded through donations and a contract with the Massachusetts Department of Public Health.
